COMPARISON OF THERAPEUTIC OUTCOME IN DIABETIC PATIEINTS ON ONLY PROKINETICS AND PROKINETICS ALONG WITH LIFE STYLE MODIFICATIONS.

Gastroparesis ,diabetic autonomic neuropathy ,Diabetes gastroparesis ,prokinetics ,

Aim: To know how effectively does the prokinetics work when strictly lifestyle modifications are followed in diabetes mellitus induced gastroparesis. Methods: It is a non-interventional observational study. This was studied for a period of 6 months in Guntur hospitals. We studied 30 patients with diabetes mellitus (11 men and 19 women whose mean age was 42 years [range 25 to 65]). All gave informed consent to participate in the study, which had been approved by the University & Hospital's ethics committee. The patients had diabetes for 5 to 20 years (mean, 11 years). All had normal serum creatinine concentrations, and their mean hemoglobin A1c value was 8.0 percent (range, 5.3 to 11.6; normal range, 3.6 to 6.4). From previous upper endoscopic studies, all the patients were known to have prolonged gastric emptying for solids, due to diabetic gastroparesis. A special scale was used for the estimation of DGP called Gastroparesis cardinal symptom index scale. Results: Total number of patients who enrolled into the study was 30 members. The 3 prokinetics are given according to the clinician prescription.20 patients were administrated with itopride where as another 10 patients were administrated with levosulpiride and acotinamide of 5 each. The GCSI index composed of 9 symptoms which is based on 3 symptom subscales of a larger upper gastrointestinal disorders–symptom severity index.. These 3 scales were selected as part of the GCSI because they assess common symptoms related to gastroparesis-nausea/vomiting, postprandial fullness/early satiety, and bloating. After the completions of 6 weeks follow-up, a total no of 11 patients were relived and 19 patients were remain unchanged as these 11 members are followed strictly lifestyle modification. The p value was obtained to be P=0.001 and results are statistically significant. Conclusion: there is significant improvement in the patients who has followed the strict life style modifications along with the drugs. Whilst alone drug shows no significant improvements
