171PHARMACIST-DRIVEN GLYCEMIC CONTROL STRATEGIES IN TYPE 2 DIABETES MELLITUSGlycemic control interventions as developed by pharmacists are essential in the management of Type 2 Diabetes Mellitus (T2DM), as they are an evidence-based, patient-driven intervention, which can maximize the effect of the treatment, enhance medication adherence, and minimise the occurrence of complications. Pharmacists deliver the necessary services, which include medication therapy management (MTM), counselling of patients on lifestyle changes with their knowledge of pharmacotherapy. These interventions are especially useful in making sure that the medications are used properly, dealing with the adherence obstacles, and improving the knowledge level of the patients about their condition. Also, pharmacists can add to the early identification and control of comorbidities, including high blood pressure and dyslipidaemia, which are likely accompaniments of T2DM. Through the implementation of pharmacists to work in multidisciplinary teams in managing diabetes, a more comprehensive and effective way of managing T2DM is implemented, which ultimately leads to better clinical outcomes, fewer hospitalization, and a less expensive healthcare system.
